Nurse Care Manager

Position Summary

The role of the Nurse Care Manager works in our Patient Support Center (PSC) in Nashville, Tennessee. The Nurse Care Manager, as part of a multidisciplinary care team, is responsible for assisting patients diagnosed with complex illnesses, recovering from a traumatic clinical event, or managing multiple clinical co-morbidities. The Nurse Care Manager leads the development and ongoing management of a comprehensive, individualized care plan with each patient. The care plan helps the patient and their family fully understand their clinical condition, manage symptoms associated with their condition, understand and navigate treatment alternatives, and address non-clinical issues that impact quality of life and outcomes.

Experience

A minimum of three (3) years of related clinical experience or equivalent, preferably in telephonic care management, case management, home care or hospice.

Duties and Responsibilities

The office-based Nurse Care Manager supports patients by providing ensuring seamless delivery of the Narus Health program for individuals and their families:

  • Receive inbound patient calls and secure patient messages
  • Perform appropriate triage and escalation of routine and urgent patient calls
  • Assess, identify, and close clinical and non-clinical gaps in patient care
  • Manage actions identified by the patient care plan
  • Conduct proactive telephonic consultations to screen for new issues, reassess the patient, update the care plan, and evaluate progress toward the goals of the care plan
  • Evaluate changes in patient-reported symptoms and conduct additional triage and screening to determine next steps
  • Support the safe transfer of the patient to their PCP, ED or hospital when clinically necessary
  • Educate patients and families on treatment options, including cost, efficacy, and side effects
  • Provide and deliver disease- and symptom-specific patient education
  • Coordinate the ordering and delivery of clinical and community services, including home health, DME and other community resources, maintain the patient’s health status and meet their goals of care
  • When appropriate, document the patient’s wishes using Advanced Care Planning documents and state specific POST/POLST forms
  • Acts as a liaison and patient advocate with the patient’s family, PCP, specialists, providers, and facilities
  • Participate in chart reviews and grand rounds to facilitate care coordination for complex or recalcitrant patients and for continuing education and quality assurance purposes
  • Communicate proactively, effectively and professionally with the entire care team, including the patient’s PCP and specialists, health plan case managers, and facility staff
Qualifications

To perform this job successfully an individual must be able to perform each essential duty satisfactorily.   • BSN required
  • Active, unrestricted RN license in the state of practice
  • A minimum of three (3) years of related clinical experience, preferably in case management, home care, hospice
  • Proficient in Microsoft Office programs, EMR/Care Management platforms, and keyboarding
  • Demonstrated knowledge of legal and ethical issues related to confidentiality, including HIPAA and state-specific privacy laws
  • Commitment to excellence and high standards
  • Ability to analyze complex data, draw conclusions, and act on that information in a timely manner
  • Ability to apply problem-solving techniques to the case management process
  • Ability to manage health crises in a calm manner
  • Excellent time management, flexibility, and organizational skills with the ability to work independently
  • Good communication, teamwork, and interpersonal skills with the ability to work with all levels of management
